HOW WE WORK

At DanielSamuel Care, we carry out our work to best meet the presenting needs of our residents (including their physical, social, psychological, and emotional needs), and mitigate relapse in their health and/or readmission into the hospital using a person-centred and partnership approach. We work collaboratively with families, professionals, people who are involved in their care and other service providers.

We provide:

Support To Manage Our Residents’ Physical And Mental Health Conditions

  • Residents will be supported and encouraged to live healthy lifestyle, to attain independence in managing their prescribed medical treatments, to understand and manage their physical health and to cope with the symptoms and challenges of their illness.
  • We provide ‘reminder system’ to enable residents to maintain contact with services and attend appointments with health and social care professionals involved in their care including GP, consultant, community psychiatric nurse, social workers, solicitors and advocates etc.
  • Support people with mental health, substance misuse, personality disorders and/or continued offending issues, address those behaviours in a way that is psychologically informed and recovery focused.
  • Enable people to achieve person-centred outcomes that address their support needs.
  • Ensure that people comply with any licence or community order in partnership with offender management agencies.
  • Support and prepare people for options through or out of the Mental Health Accommodation Pathway.

Move-On Support

Our skilled and experienced staff support our residents to develop their move-on support plan. The scope of move-on support delivered is agreed with the resident, but it will typically include:

  • Assistance with understanding and maintaining their tenancy.
  • Advice, advocacy and liaison with other housing related agencies.
  • Resettlement (3 weeks) support at the point of move-on to independent accommodation.
  • Support and integration with immediate neighbourhood and access to local facilities.
  • Advice and practical support for setting up, efficient use and maintenance of utilities and services such as electricity, gas, water, internet, satellite TV, and other essential modern facilities in the residence.
  • Advice and support for residents to keep their personal spaces and other communal areas including the garden and surrounding areas tidy and clean.

Support With Accessing Community Resources And Services

  • We will register our residents with the local primary healthcare services and accompany them to their appointments where necessary.
  • We encourage our residents to enrol for educational, vocational, apprenticeship or other learning activities of their interest, and where necessary we accompany them to settle them into productive and fulfilling routine to avoid idleness and its accompanying consequences.
  • We encourage and signpost our residents to develop structured lifestyle and where necessary we accompany them to activities centres such as work placement, voluntary workplace, allotment or gardening, sporting activities.

Support With Social Activities And Companionship

  • We encourage and support our residents to be creative, to engage with appropriate local social networks and to engage and participate in activities that are recognised to be of help for them to cultivate and maintain healthy social lifestyle.
  • We organise seasonal social activities such as excursions, summer outings, cycling clubs, eating out and other group activities that are aimed to promote people’s health and well-being and engaging them in local community activities.
  • For those of our residents who are interested, we support them to attend and participate in religious activities of their interest.

Support With Practical Activities Including Support To Attain Personal Independence.

  • We therefore encourage our residents to carry out basic activities of daily living. Where necessary however, we help/assist our residents to do their shopping for groceries, prepare and cook their meals. We encourage healthy eating and therefore we always make fresh fruits available free of charge.
  • We encourage residents to take responsibility for and managing their finances (e.g. Paying bills, liaison with benefit agencies). We will also encourage for our residents to have support in the form of appointeeship if considered necessary for them to be on top of their finances and budgeting.
  • We encourage and support our residents to develop confidence to communicate with essential third parties (e.g. form filling, emailing, telephone calls) and to understand and maintain confidentiality about their own affairs.
  • We work with our residents to maintain safe and clean environment as part of our care and support package to enable them to learn to keep clean environment when they resume full independent living.
